Go to: Member Login and click on Medical / Dental / Pharmacy Login. Click on the option to "Sign Up Now" and follow the on-screen instructions. You will need to have your ID card available.

Your dental provider can submit a pre-determination request, listing the procedures you are having done. This will be processed and a letter sent to both you and your provider with the allowances for any covered services.

Your provider will need to submit a letter of medical necessity and medical documentation to support the services rendered. Once received, a review is done to determine if the services are considered medically necessary.
